Employees need constant training in order to avoid situations like that

In some fields, this idea may be just fine. If your employee only has one task and the task never changes, perhaps it is true that there is nothing more to learn. However, in the vast majority of other cases, ongoing employee training is essential. For example, if you introduce a new system, you need to make sure that your employees know how to use it properly.
